Running Ubuntu 9.04, i686 version on an AMD Athlon 2500+, using Upgrade Manager to install linux-image-2.6.28-18-generic (2.6.28-18.59). During the install, I was asked what I wanted to do about the menu.lst. After choosing to accept the maintainer's version, I walked away & when I came back there was an indication that several packages had failed. Here is the relevant section of /var/log/apt/term.log:

First time I've seen this in quite a few kernel updates and several major versions of Ubuntu.
