Thank you for taking the time to report this bug and help make Ubuntu better. Please provide the output of the following commands as separate attachments, both with and without AC power:

  gnome-power-bugreport.sh
  hal-find-by-capability --capability "battery" | xargs -n 1 hal-device
  hal-find-by-capability --capability "ac_adapter" | xargs -n 1 hal-device

Please also specifically describe the contents of the tooltip in both conditions (provide screenshots if possible).

Thanks in advance.

Attachments added. screenshots would be redundant - the status icon simply doesn't change.

Ok, but a description of the exact contents of the tooltip would be nice. I don't use a laptop, so i never see this icon and I don't know what information it contains.

The information provided by HAL all looks consistent. When running from battery:

battery.rechargeable.is_charging = false
battery.rechargeable.is_discharging = true
ac_adapter.present = false

When running from AC:

battery.rechargeable.is_charging = false
battery.rechargeable.is_discharging = false
ac_adapter.present = true
battery.charge_level.percentage = 100

It seems that in your second case, the battery is fully charged. It would be good to see the same set of information with the AC adpater present and the battery charging (ie, charge_level < 100)
