You could try to re-install orca and the accessibility stack packages, and then as an alternative, remove them and purge their configurationfiles, and then re-install.
The packages you want to reinstall are gnome-orca, at-spi2-core, libatspi2.0-0, gir1.2-atspi-2.0, libatk-adaptor, libatk-bridge2.0-0, python3-pyatspi.
