Can you try running:

gnome-control-center credentials

at a terminal and pasting any error messages that are printed into this bug report? Additionally, can you try upgrading gnome-control-center to the latest version (I have 1:3.4.2-0ubuntu14 but you seem to have 1:3.4.2-0ubuntu13).
