i'm trying to fix the ABBA deadlock by introducing a new
lock into dlopen that protects tls state and gets released
before init functions are run. using that lock at thread
creation would fix the issue.

this only requires a small amount of changes, but it seems
to be difficult to ensure that the new lock is released on
all failure paths within _dl_open_worker (which sometimes
uses longjmp for error handling).
