Form buttons should be rendered using Ubuntu's selected theme. Each form item (checkboxes too) on this site: http://www.w3.org/2000/07/8378/xhtml/media-types/test.xml#forms.heading should not be displayed in Win98-style.
In fact, on 404 error pages displayed by browser, "Try again" button is displayed correctly. However, as far as I know, the button declaration is written in XUL with this code so it's not ordinary form button:
