I very much like the idea behind comment 110 but would expand upon it slightly. Use a bar akin to remember password. Allow the user to save a default action and should action be anything other than "Save as..." a bar come across indicating possible actions.

If the action is anything other than save as a simple bit of text akin to comment 110:

"The server [insert domain] recommends saving this file, your default action is [insert default action].

Along with buttons: "Save as...", "Change action", "Dismiss"
Dismiss = close message continue action
Save as = open save as dialog box
Change action = open dialog for selecting a one time action with an option to make it the default action, take the new action

In doing so it should not attempt to stop or delay the execution of the default action. Akin to if you click a link the browser continues to load until given new instructions. This then has the added benefit of being able to open and save a document simultaneously.
