I have encountered the same bug with Firefox 3.5.2 on Windovs Vista SP2.

Summary:
1. The bug could be related to Firebug and Live HTTP Headers plug-ins.
2. In some cases this workaround works: Close the tab with the upload form. Undo close tab.
