Chad, it should be enough to delete the /etc/udev/rules.d/S92-dmraid.rules and then reinstall the dmraid package so that
/usr/share/initramfs-tools/hooks/dmraid and /usr/share/initramfs-tools/scripts/local-top/dmraid are restored.
