I have this issue too, I use two keyboard layouts: US and DE (German). Although US is at the top of the list (I believe this is how the default is set), sometimes the layout changes randomly to DE; usually when switching apps (although "Separate layout for each window" is disabled).

I believe the older Gnome versions had a "default" checkbox for each entry in the layout list.
