I experience the same problem.
Gusty. Compiz on Extra. Nvidia GeForce Go 7400.
I had the black window bug that seems to be common with the nvidia cards and compiz. Resolved that by enabling direct rendering.
The bug behaves just as discribed above. Title bar disappears on every seventh title bar update. Comes back the next time it's updates.
