I suspect W: means warning. It gave a bunch of warnings about duplicate entries, then, with the same W:, told me "You may want to run apt-get update to correct these problems". Yes, this is a problem, and yes, I do want to run apt-get update, which will "correct these problems". It doesn't, so that's what's wrong.
As for "E: Some index files failed to download, they have been ignored, or old ones used instead.", I believe it simply gave that because it couldn't resolve some addresses, which isn't relevant to the issue.
