I based that on this:

W: Duplicate sources.list entry http://security.ubuntu.com gutsy-security/universe Packages (/var/lib/apt/lists/security.ubuntu.com_ubuntu_dists_gutsy-security_universe_binary-i386_Packages)
W: Duplicate sources.list entry http://security.ubuntu.com gutsy-security/main Packages (/var/lib/apt/lists/security.ubuntu.com_ubuntu_dists_gutsy-security_main_binary-i386_Packages)
W: Duplicate sources.list entry http://security.ubuntu.com gutsy-security/multiverse Packages (/var/lib/apt/lists/security.ubuntu.com_ubuntu_dists_gutsy-security_multiverse_binary-i386_Packages)
W: Duplicate sources.list entry http://security.ubuntu.com gutsy-security/restricted Packages (/var/lib/apt/lists/security.ubuntu.com_ubuntu_dists_gutsy-security_restricted_binary-i386_Packages)
W: You may want to run apt-get update to correct these problems

I suspect W: means warning. It gave a bunch of warnings about duplicate entries, then, with the same W:, told me "You may want to run apt-get update to correct these problems". Yes, this is a problem, and yes, I do want to run apt-get update, which will "correct these problems". It doesn't, so that's what's wrong.

As for "E: Some index files failed to download, they have been ignored, or old ones used instead.", I believe it simply gave that because it couldn't resolve some addresses, which isn't relevant to the issue.
