akonadi-kde and akonadi-server are all within the monolithic package ... and that doesn't depend on mysql-server because mysql-server is incredibly big and power draining and and and.
Long story short, if you want to use akonadi, then install mysql-server and ensure the apparmor profile for it is accurate (or that apparmor is deactivated).

The latter probably can be addressed in the way the official packages do (i.e. hardlinking mysql-akonadi to the mysql binary). Depending on mysql-server possibly could be done in kdepim, not sure if that improves the situation.

In any case: this is only going to happen in 9.04, the 8.10 mysql package structure just doesn't fit the akonadi usecase.
