Missing title bars can be fixed as follows:
Go to => Preferences->Advanced Desktop....
Select Effects->"Windows Decoration"

If it does not work:
- turn off the windows decoration
- close all applications
- restart window manager (Ctrl+Alt+Backspace - will kill all ur applications)
- turn on the windows decoration

I used the 2nd approach, so not sure abt the 1st one
