I'm not sure yet what the common denominator is to reproduce this bug, but it seems to often happen on phone boot where changing volume, either by screen touch or the phone's volume buttons, /does/ correctly change the volume but does /not/ pop up the org.freedesktop
Since the problem seems fixes itself if you run "restart indicator-sound", so it may be a timing issue with i-sound starting up at the same time as unity-notifications and querying its abilities before unity-notifications is ready for business.
