I moved the drive and sas controller to another system, just to make sure there wasn't something wrong with the machine itself that I was testing on.
I got the same results as before once the installer finished up and the server tried to boot again. I definitely have an 'ubuntu' entry in my efi boot menu, but we fail to boot the OS.
The EFI boot entry on both machines does seem to have a path to grubx64.efi. For example, in my latest attempt, this is what I see if I look at the device path for the ubuntu EFI menu entry:
