There are 32 pages of comments dating from year 2000 and yours are only from 2008. It was plain obvious that I was referring to the older comments. But I guess not obvious enough for you. Anyway moving on.

If the mail is coming from a mailing list you should be replying to the mailing list email and not including other recipients unless explicitly requested by them (e.g: "Please CC me I am not subscribed to the list"). At least this is my understanding how mailing lists work and this is how would I expect a Reply To All to work in this case. "All" will mean "All subscribers on this list".
If the mail is not coming from a list then "All" will just "Everyone from To and CC".
