From the output it looks like we have the LVM physical volume for what will ultimately be the rootfs set up but not the logical volume (I don't think we can tell from this output if the VG is set up). What happens if you run vgs, vgchange -a, that sort of thing from the prompt?

I don't see anything in any initramfs script that explicitly loops around multipath / lvm2 / cryptsetup but shouldn't udev handle this these days? Maybe we can make udev super verbose somehow like adding udev.log_priority=DEBUG (or is that rd.udev.log_priority=DEBUG?) to the kernel commandline.
