Comment 2 for bug 689389

I've styled to match the wireframe. It seems a bit weird that it uses a different layout to the other forms (ie. label above field, rather than left of field). Also, I'm assuming the "(used in SSO)" is not part of the field label ("Full name (used in SSO):" shown on wireframe). Let me know if this isn't the case.

Also, I left the bank details off, as the wireframe has a question against it ('why do we need this here'), and also left the "Read and sign our contract thing", as the dev has signed this when first using myapps. I'll upload the wireframe with a second attachment.
