2) Otherwise treat the "suggests" as suggestions,
i.e. ask the user if they want to install each suggest.

I would favour presenting a list of all the suggests together, with name of the package suggesting each, and preferably a one-line comment indicating what each suggest provides.
Then let the user decide for each, "yes", "no", or "ask me later".
The latter would be saved to be asked automatically on subsequent updates.

This would probably involve some work to implement, but I think that this is the "right way" to treat suggests.
