With the ellipse tool, set 'Fill Style' to either "Outline Shape" or "Fill and Outline Shape"; you can use any 'Shape Type' but it'll be most obvious when set to "Ellipse" The easiest way to see this is select a high-contrast color (e.g. black on a white canvas) and set your brush width to something like 50. From there, simply draw an elipse and the issue should become extremely apparent—your "ellipse" will look more like a low-polygon oval from an old late-90s/early-2000s video game (see attached screenshot).
