Can you enable coredumps for mysql and provide backtrace from that. You can use https://gist.github.com/ronin13/8d403c2f88826fcc7aba as the gdb script for it.

Also, general log (decoded binlog if you have it), my.cnf and any steps to reproduce this would be great.

Also, do you run any prepared statements? http://bugs.mysql.com/bug.php?id=42041 has some details and workarounds for 1615 (ER_NEED_REPREPARE) - such as increasing table_definition_cache.
