Jervin, I believe you are hitting this case: https://bugs.launchpad.net/codership-mysql/+bug/1130888 it deals with cascading foreign key constraints.

The bug troubleshooted here, is about parallel applying of replication events for table with no primary key. I have hard time getting this issue reproduced. I'm trying with Dan's parameters and just started a test with new scenario, it sure does not surface easily for me.
