I don't see why this bug has been set to "Confirmed." It still looks like a system configuration issue, and a Wishlist documentation bug at best.
Nil, can you post the complete /etc/pam.d/mysqld files you used?
Some superficial googling turned up some advice that, if I'm not misunderstanding it, all auth/account stacks must be finished by pam_permit/pam_deny in the configs.
