I think that is the problem, PAM plugin should work without the need to start MYSQL as root :-). At least this should be doc bug based on comment #8, if it is *expected* to work without pam_permit.so, that takes as back to why the plugin does not work with CentOS 5 and we already have a repeatable test case right?
