Screen reader is not triggered during live cd and installer when on DVD on hardware

Bug Description
STEPS:
1. Download the 15.10 iso
2. Burn to dvd
3. Boot it on a non uefi system
4. Tap a button when the keyboard icon appears
5. Select a language
6. Hit f5 select screenreader
7. Hit enter
8. Move the menu item down to install
9. Get to the first page of the installer
EXPECTED:
I expect to hear orca announce the page
ACTUAL:
I get no audible feedback until I am in the installed system
TEMP WORKAROUND:
Screen reader seems to behave fine from pendrive, so that might be the potential solution as a stop gap.
Also I couldn't reproduce the issue with a virtual cd in kvm, and 14.04 daily live dvd on the same hardware the screenreader works

It appears that the issue is the cd menu is not setting the screenreader correctly on the cd, however is doing so on pendrives. However if you trigger the screen reader in Ubiquity after the sound of the drums it then works as expected.
