Is it the only hypervisor that has this FQDN/hostname discrepancy? It should not really matter unless the code paths actually start caring when Cinder is used but this is a matter for Nova.
I think you did too many CONTROLLER_IP replacements, i.e. some of these (notably my_ip) should have local ip instead.
