Paramiko is parallel safe, eventlet by breaking all the standard tools breaks it is all :) but tempest isn't eventlet based AIUI, so we can ignore that.
Is this test using config drive or metadata server? My first suspicion would be failed cloud-init in the VM leading to no creds - so either a metadata server failure, or config drive not being attached properly or some such. The block device being attached at the same time could suggest an interaction w/config drive.
