@Slawek: I started looking at it again yesterday, and the change that Rodolfo did fixed it the kolla folks. There is a patch here: https://review.opendev.org/#/c/732081/ for ovsdbapp that they said also fixed the issue. Though I was never able to recreate the issue myself and the existing code really shouldn't allow individual indexes to be created twice anyway.

Since we have two different things that make the problem go away, we can probably go ahead and close it. The mystery still bugs me a bit.
