From your logs, it seems this 'unicat-to-tun' rule is missing between the good and the bad.

Is this port with MAC fa:16:3e:bf:ae:93 still alive and is seen in other nodes.
Is this a VM port or a non VM port.

Was this rule only missing in the Controller Node and not on other Compute Nodes.

If it is only on the controller, was the controller configured for HA?

If it is a VM port did this VM ever migrate or at least this port went down and came up. ( I mean if the VM was shutdown and then restarted or so).
When this event happened, did the neutron server or the neutron-agent went down and came up.
