ok, the fix will be committed in moblin-applets GASTON 0.53. The .realplayer directory produces the exact same thumbnail directories and files, but to be safe, I do a recursive directory search in /home/ume for all instances of "thumbnails/largest". For example if the directory /home/ume/.realwinampvlcintervideo/junk/stuff/thumbnails/largest exists it will be scanned for image files that match the thumbnail naming format. If there are multiple thumbnail/largest directories, all are scanned, and if there are any thumbnail duplicates only the first one found is added to the wallpaper list. I tried this on the 20080407_q1u_installusb.img target and it worked well.
