If you are a site admin, you can add users to an institution circumventing the invitation process, cf. http://manual.mahara.org/en/latest/site_admin/institutions.html#members Alternatively, if you only want to add one person to an institution, you can do so on the accounts page, cf. http://manual.mahara.org/en/latest/site_admin/users.html#user-account-settings

In general, I think it is by design that institution admins cannot add users in bulk as they should be able to control which institution they want to join. However, in Mahara 1.5, where we have implemented a few features for Mahara sites that run multiple institutions and thus use Mahara in an educational setting, e.g. many schools using it, there is a feature where you as admin can add users without inviting them first, cf. http://manual.mahara.org/en/moving_accounts/site_administration/institution_members.html#people-who-have-left-a-given-institution
