Bug Description
When you use SAML, you can define site admin and staff as well as institution admin and staff roles to give to account holders automatically.
Currently, the site admin and site staff roles update when a person logs in. However, that is not happening for institution admin and institution staff on subsequent logins unless the config.php variable usersuniquebyus
