Won't be fantastic on production until it's fully upgraded to PostgreSQL 9.1 (it seems that 8.4 doesn't always know to skip most of the index when the nulls are first and there's an IS NOT NULL constraint), but it's fine on qastaging and vaguely adequate on production.
