I propose to use target milestones and release series instead of (2) and (3): when a milestone becomes inactive, the bug can be considered "released" for the specified release series or the active ones.
Also, instead of showing "Fixed" every time (which can be ambigous), I think that it would be better to display "Fix committed" or "Fix released" automatically, but keeping a single item in the list of +ediststatus.
