Looks like an issue with glance or nova not able to download the image from glance, is the image active? What kind of storage is being used? Are you able to manually download the image? If using ceph check if all osd are UP, pgs active-clean (docker exec ceph_mon ceph -s), the image properly stored in the backend used.
