keystone-manage does not look for any keystone.conf except the one in ~/keystone/etc/keystone.conf not can --config-file be specified on the command line.
keystone-manage's behaviour should match keystone-all and a) allow a config file on the command-line and b) look in common locations (i.e. /etc/keystone, ~/.keystone).
Also, rename etc/keystone.conf to /etc/keystone.conf.sample so keystone-manage does not find an unconfigured file.
