The title bar menu is something is something which comes from KDE's window management and is the same in KDE 3 and 4. It is available in every application and not specific to Konsole. It just so happens that it can be used to fulfill the same need.

This is fixed in the current version of Konsole to use Ctrl+Shift+B instead - definitely in 4.3 and possibly in an earlier version.
Configure Shortcuts changes should be permanent but I believe there was a bug which affected only the 'Add Bookmark' action.
