Sorry, the channel information can be confusing. So in the original situation, Charm A was at rev 140 and Charm B was at rev 141.

Then the update in step 1 moved charm A to rev 141 and charm B to rev 145. The problem is that although juju gave the correct status output, charm A contained code that should not be present in rev 141.
