OK, after a day of slow reproductions and extra logging, I'm not any further than Nate was at the end of the day, but I have confirmed that something weird is going on.

http://pastebin.ubuntu.com/17085730/ contains some painstakingly formatted log output.

The worst bit of this bug is that it is very weirdly intermittent. At the start of the transaction build process I output what was stored in the DB (using a bson.M to avoid assumptions - perhaps should have used a bson.D, but c'est la vie).

All the transaction asserts appear to be met, but for some weird reason it fails.

I have nothing.
