According to the Instructions[1] shared in the Download Page[2], user can boot the Image from a USB drive. This is helpful incase the platform doesn't have an internal disk.

In any case, the issue occurs if an additional USB disk is present at the time of booting. As you mentioned in #3, it is because the root is still set to 'hd0,gpt3' even though the sequence changes. I believe the root should be set based on the UUID of the device from which it boots, something like: root=UUID=<uuid_of_boot_device>
