Incorrect path for dynamic or linked offset

Bug Description
I have been having trouble with these offsets when there is a mixture of lines and curves in a path. Oddly, cutting and pasting the path to a new document sometimes improves the situation.
However, here is a simplified single path with just a few lines and curves which demonstrates that the offset path cuts inside where it shouldn't.
Inkscape 0.91 on Ubuntu 14.04LTS

Confirmed on Windows 7, Inkscape 0.91 and trunk rev. 14751.
