    I am using the original file rect0485.svg from comment 4. It is possible that you may have encountered Case 2b instead of Case 2a which is a bit simpler to deal with. I think the main point is that the main Inkscape code uses the bottom left hand corner of the document as the origin when toggling Page Size units. If you load the file and then toggle the page size units repeatedly between size A4 and size Letter, you will see that the bottom left corner remains fixed as the origin. Since the bottom left is the origin, it is reasonable to expect that the Python extension will also treat it as the origin as well, in order to remain consistent with what Inkscape has been doing. However that is not the case. The origin at the bottom left shifts when applying the extension, which is unexpected.
