It seems most likely that this situation only occurs when the user enters a userID that is case-sensitive and does not match the case also specified to the server. Firefox then tries to "help" the user by selecting the bad ID that was previously entered.
If there were some easy way to get Firefox to forget a particular entry, I think this would solve the problem. I would love to be able to get the list that Firefox is offering, then point to the bad item and hit delete.
