Record user who creates patron record

We often encounter data entry errors in patron accounts where it would be useful to determine who created the account so that we can follow up with additional training.

This would necessitate adding a new field to actor.usr and automatically adding the staff user id to that field upon account creation, but I'm unsure what would be the best way to handle accounts that already exist. Would it be better to leave that field null on existing accounts, or to have an upgrade script that adds something (1?) to the fields on those accounts?

Adding an automatic staff user id field won't necessarily report on who created the account since not every libraries requires staff to log out at the end of their shift and some libraries use one login for all circ staff. Adding a field that requires staff to enter their initials before saving will accurately report who creates patron accounts. My library uses the Other Phone field for this purpose.
