Double checkboxes in batch modify holds modal

Bug Description

When batch modifying holds, any option displayed as a checkbox requires the user to check both the "activate column editing" box to the left of the label as well as the option-specific checkbox to the right. Furthermore, the "activate column editing" inputs don't have labels, only title attributes, which screen readers do not announce.

One checkbox should do the job here, and every input needs a proper <label>.

Spotted in 3.9 but probably goes back further.

Agreed the layout could be improved.

However, the 2nd checkbox (or similar control) is needed to specify which values should be applied to the batch of holds. Without it, an empty/unchecked "hold is suspended" checkbox (for example) would activate all of the holds in the batch, even if the user didn't intend to change the value.

We need some way of indicating which values we want to apply to the batch.
