Labels not associated with checkboxes when placing metarecord hold in staff client

Bug Description

When placing a metarecord hold in the staff client there are checkboxes to select the desired formats and languages. The associated labels neither contain the relevant checkbox nor are they associated by id, so web browser do not associate them with the checkbox and the label therefore cannot be used to control the checkbox. This is both an accessibility and a usability issue.

Observed in Evergreen 3-7-3.

Thanks, Stephanie! Pushed to rel_3_10 and above, along with a quick follow-up to prevent the checkbox accessible names from being "Large Print Book Large Print Book" and the like.
