Order the survey editor fields in a more reasonable way

Bug Description

Jennifer Pringle mentioned this on bug 1879517: "The Survey End Date/Time field comes before the Survey Start Date/Time when creating or editing a survey which lends itself to people entering the opposite dates by accident"

I'm breaking it out into its own bug, with a branch forthcoming.

Steps to reproduce:

1. Press Admin > Local Admin
2. Press Surveys
3. Create a new survey.
4. Note that the end date comes before the start date.

Alternatively:
1. Press Admin > Local Admin
2. Press Surveys
3. Select a survey and edit selected.
4. Note that the fields are also in an unintuitive order.
