Bootstrap OPAC: implement course materials display

Per bug 1849212, the Bootstrap OPAC should implement the new course materials functionality, including:

- course browse
- course search
- displaying when an item is assigned to a course on the record details and search results pages

It looks good, Jane. The courses appear in the search results and in the bib records. The search courses, and browse courses searches work well. The course details look great.

I found one oddity. If you click on the 'Search Courses' menu, a duplicate menu displays under they main menu with a drop-down for basket options. See attached.

I note that some styling work remains to be done. For example, the course search form should be adjusted to use the same styling as the cataloging advanced search form. However, since I believe that getting the functionality available in the new skin outweighs design consistency, I'll just open a new bug.
