style course materials pages in the Bootstrap OPAC

Bug Description

Course materials search and display functionality was added to the Bootstrap OPAC theme in bug 1895678. However, pages like the course search form are using TPAC styles, not Bootstrap styles, and should be updated.

Evergreen 3.11.1

This is causing search issues as the boxes are small and you can easily mistake "Course Name" for "Course Number" and vice versa as all that displays is "Course Na" and "Course Nu" .
